Course aim

Students will advance their knowledge in the management of older people with chronic and complex health problems in a range of settings.

Course content

Assess, plan and manage care around the individual, including family; differentiate between disability, frailty, co-morbidity, complexity and chronic conditions; models of care including transitional care, continuity of care/discharge planning, rehabilitation/restorative functional care/palliative care and end of life care applying the best research to practice; exploring range of evidence required; health education/health promotion/disease management/preventing other diseases; intra/inter-professional role and collaboration; multi and trans disciplinary care; dementia; mental health; continence management; wound management; quality use of provision of care for older people in acute, community, home and residential care settings, technological/innovative strategies.
