New student declaration form
In accordance with the University of South Australia's Clinical & Health Sciences (Nursing & Midwifery) Fitness to Practice in Clinical Placement Guidelines and Procedures, all students are required to review the Guidelines and Procedures and confirm their ability to meet the Fitness to Practice in Clinical Placement Requirement Statements at the commencement of each academic year.
These statements identify the essential abilities, knowledge and skills needed to achieve the core learning outcomes of the nursing and midwifery courses that lead to professional registration with the Nursing and Midwifery Board of Australia (NMBA) and Australian Health Practitioner Regulation Agency (AHPRA).
There are eight domains of the Fitness to Practice in Clinical Placement Requirement Statements, including:
Students who cannot provide confirmation on the points above, or require further information before they can provide this confirmation, should contact UniSA Clinical & Health Sciences (Nursing & Midwifery) by emailing NRCConditionalEvidence@unisa.edu.au with the subject line 'Fitness to Practice Requirements'.
By submitting this form I confirm my ability to meet the Fitness to Practice in Clinical Placement Requirement Statements. If my situation changes, it is my responsibility to inform the Program Director in Clinical & Health Sciences (Nursing & Midwifery) as soon as possible.
